HP Sports Waiver and Release Agreement for Minors (under 18 years)

By continuing, I, the parent/legal guardian, consent for my minor child to participate in activities conducted by HP Sports.  I acknowledge that I fully understand that his or her participation may involve risk of serious injury, illness, or death, including losses which may result not only from his or her own actions, inaction or negligence, but also from the actions, inaction, or negligence of others, the condition of the facilities, equipment, or areas where the activity is being conducted, and/or the rules of play of this type of activity.
 
Knowing and understanding the risks involved with participation in the activity, I hereby voluntarily and willingly assume full and complete responsibility for all losses and damages, including injury, illness, and death, resulting from allowing my minor child to participate in the activity.  I agree I am financially responsible for any losses and damages resulting from his or her participation in the activity.

I certify that my minor child is in good health and has no medical condition preventing his or her safe participation in this activity. I agree to use his or her personal medical insurance as the primary medical coverage if accident, injury, or illness occurs. I also consent to emergency medical treatment for him or her in the event such care is required.

I grant permission to HP Sports to use any photographs taken of my minor child during sports activities to be used for publicity purposes, including the HP Sports website, brochures, and other publications printed by HP Sports.

In consideration for allowing my minor child to participate in the activity, I hereby waive all claims or causes of action against the Owner, Program Directors, and volunteers arising out of his or her participation in the activity and hereby release the Owner, Program Directors, and volunteers from all liability in connection therewith...
